Supporting Your Every Effort Toward Recovery Serving in the home or in the community
Each person is responsible for his or her own recovery. We are here to support every effort you make!
As your recovery partner, your personal case manager will offer:
- Hope that recovery is possible for every person
- Partnership and support as you take each step of your recovery journey
Your personal case manager and recovery partner will visit with you at least twice each month in the community. During your visit, you can review and adjust together your plan to build your personal recovery skills. You can also review your plan to get the resources you need to support your recovery. You can decide about any needed or wanted service changes. You can build skills for a better life!
Case management and related PSR-I* services give the opportunity to learn and practice real-life skills for a successful everyday life! Your possibilities are endless!
For example, you can:
- Identify your strengths, abilities, and preferences
- Learn to set personal goals and the steps to reach them
You can also:
- Identify any barriers to recovery
- Identify past triggers of struggles and partner with your case manager to overcome them
You can learn and practice skills to:
- Manage symptoms
- Cope with stress
- Improve relationships with family, friends, and others
- Become more employable
- Prevent crisis
- Overcome any challenge
Your case manager can also support you as you:
- Take care of your health
- Find or maintain stable housing
- Apply for any available benefits such as food stamps, social security, or VA
- Locate any other needed resources (food, medication, clothing, etc.)
- Make transportation arrangements
- Complete needed applications
- Learn to make and keep appointments
- Build or strengthen your network of positive community support (family, friends, and others)
- Live a life of purpose and contribution in your community
Your case manager is your partner to achieve any positive personal goal!

Team-Based Services:
Assertive Community Treatment (ACT) Team
The River Edge Assertive Community Treatment (ACT) Team is a multidisciplinary mobile team – including a prescriber and a nurse - that provides clinical and wrap-around services to adult residents of Macon-Bibb and Baldwin counties whose clinical needs exceed traditional outpatient services.
Intensive Case Management
In conjunction with traditional outpatient services, the River Edge Intensive Case Management Team provides community-based, wrap-around assistance to individuals who need a bit more help negotiating resources and developing community living skills as they develop lives of recovery.
