Community Support Services consist of rehabilitative, environmental support, and case management services designed to assist clients in gaining access to community agencies. Services are intended to promote recovery and improve interaction within the community for adults, children, adolescents and their families. Community Support Specialists meet with individuals in their homes, schools, or community settings to ensure that basic needs are met and to improve self-management of symptoms.
The Community Support Services Division has a Client Benefit Specialist to provide assistance with Social Security applications, reviews, and /or denial process. The Client Benefit Specialist will meet with you to help complete all necessary forms, and compile medical documentation, help answer questions, and provide assistance with interviews and appointment dates.
Some individuals who receive Social Security disability benefits may also be eligible for Medicaid benefits. The Client Benefit Specialist will provide assistance to determine your eligibility.
The Client Benefit Specialist is available Monday-Friday, 8:30 a.m. to 5:00 p.m.
Other services are available Monday-Friday, 8:30 A.M. to 7:00 P.M. and on weekends as needed. Services are available 24 hours through the after-hours contact number.
